We are sorry that you are concerned with your rose(s). As we are passionate in helping you get the most from your roses, we need to ensure we are offering appropriate assistance and accurate advice.

Please complete the below form with as much detail as possible and return to us.

Do not answer any questions with ‘as instructed’. If more than one rose is causing you concern, specify in your responses if the care varied between the plants.

* Detail how the soil / garden bed was prepared. Include products used.

* Did you prepare the potting mix yourself or was it purchased pre-made?

* What type and brand did you purchase?

* What did you use to make the potting mix? (please list all material and products with amount used).

* When did you make the potting mix?

* What is the soil pH specifically in the position where the rose(s) is planted?

* Has the plant produced any growth since being planted?

* When did you first notice the growth deteriorating?

* How many hours of direct sunlight is the plant(s) receiving?

* Have you used any garden products in or around the bed since planting? (including lawn care products)
